POSITION SUMMARY Mentors provide supportive and psycho-educational interventions to youth clients who are engaged in behavioral health treatment at Centennial. Interventions may include recreational outings, small group activities with other mentor youth, and one-on-one projects. The Mentor works as part of a multi-disciplinary team to support the client's progress toward treatment goals. Mentoring activities may take place in the local office, in community settings, in homes or in schools. The Mentor serves as a positive role-model and skills coach.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Meet with assigned youth 1-3 hours weekly, facilitating planned interventions which align with treatment goals. Interventions may be provided individually or in small groups.
2. Communicate regularly with other members of the treatment team to design appropriate activities which are personalized to each youth client.
3. Document all activities in the electronic health record in a timely manner and in accordance with standard documentation requirements.
4. Conduct Brief Screening interviews with youth clients and their care-givers.
5. Make appropriate internal and external referrals to assist youth clients and their families in obtaining needed goods and services that address social determinants of health.
6. Complete all required training as assigned by agency and supervisor.
7. Have a valid CO driver's license and be able to transport youth in agency vehicles.
8. Attend regular supervision sessions and other required agency meetings.

JOB REQUIREMENTS

EDUCATION Bachelor's Degree in psychology, social work, or other human services field.

ABILITIES, KNOWLEDGE, SKILLS 1. The successful candidate will have excellent interpersonal skills and the ability to engage, motivate and support youth clients. 2. Ability to serve a wide range of individuals, including culturally diverse populations.
